Notice of Privacy Practices

Our responsibilities
Dental Care of Woodstock is required by law to maintain the privacy and security of protected health information, provide this notice of our legal duties and privacy practices, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information.
How we may use and disclose information
We may use or disclose protected health information as permitted or required by law, including:
- Treatment: to provide, coordinate, or manage dental care and communicate with other treating professionals.
- Payment: to bill, obtain payment, verify benefits, and communicate with health plans.
- Healthcare operations: for quality review, training, licensing, compliance, and practice administration.
- Appointments and services: to contact you about appointments, treatment alternatives, and health-related services.
- Public health and safety: for legally authorized reporting, preventing serious threats, and workers' compensation.
- Legal requirements: when required for oversight, law enforcement, judicial proceedings, or other duties imposed by law.
Other uses and disclosures require your written authorization when the law requires it. You may revoke an authorization in writing, except to the extent we have already relied on it. We do not sell protected health information or use it for marketing when authorization is required without obtaining that authorization.
Your rights
Subject to legal limitations, you may:
- Inspect or obtain an electronic or paper copy of your health and billing records.
- Ask us to correct information you believe is incorrect or incomplete.
- Request confidential communications at a particular address or by a particular method.
- Ask us to limit certain uses or disclosures. We are not always required to agree, except where the law requires.
- Request a list of certain disclosures made during the applicable legal period.
- Receive a paper copy of this notice at any time.
- Choose someone with legal authority to act for you.
Complaints
You may complain to the practice using the contact information below or to the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.
Changes to this notice
We may change this notice and make the revised notice effective for health information we already maintain and information we receive in the future. The current notice will be available at the office and on this website.
